REGISTERED NURSE WITH INTEREST IN GERIATRICS

Are you a Registered Nurse who is interested in improving care pathways in primary care for supporting older adults diagnosed with dementia?  The person in this two-year, grant-funded position will help refine and pilot tools and workflow protocols for early detection, diagnosis, and referral processes for patients diagnosed with Alzheimer’s Disease and related Dementias. You will have the support of mission-driven colleagues who appreciate the opportunity to support older adult patients and their caregivers in rural communities.  The ideal candidate will possess experience in the provision of care to older adults, enjoy process improvement work, be skilled in data collection, and have a history of developing collaborative relationships with internal and external partners.  The role will offer the opportunity to travel to health center sites in western and central Maine as well as remote work from home.
