Are you a Registered Nurse who is interested in improving care pathways in primary care for supporting older adults diagnosed with dementia? The person in this two-year, grant-funded position will help refine and pilot tools and workflow protocols for early detection, diagnosis, and referral processes for patients diagnosed with Alzheimer’s Disease and related Dementias. You will have the support of mission-driven colleagues who appreciate the opportunity to support older adult patients and their caregivers in rural communities. The ideal candidate will possess experience in the provision of care to older adults, enjoy process improvement work, be skilled in data collection, and have a history of developing collaborative relationships with internal and external partners. The role will offer the opportunity to travel to health center sites in western and central Maine as well as remote work from home. Belgrade Regional Health Center, founded in 1977, is a part of HealthReach Community Health Centers – a Federally Qualified Health Center consisting of twelve community health centers located across Central and Western Maine. Located in the beautiful Belgrade Lakes region of central Maine, this position would place you amongst the picturesque chain of seven sparkling lakes and abundant streams.
