HealthReach Community Health Center seeks a float Family Nurse Practitioner or Physician Assistant to provide primary care to patients of all ages. HealthReach Community Health Centers has 11 practices thought Central and Western Maine. The selected candidate will provide outpatient medical care with integrated behavioral health services to individuals and families in our NCQA certified PCMH outpatient practices.
