Western Maine Family Health Center (Livermore Falls) seeks a full-time Physician to provide primary care and preventive services in a family practice. As a NCQA-Patient-Centered Medical Home, we offer accessible, high-quality healthcare with integrated behavioral health services focused on the patient’s care experience, which include routine physicals, chronic disease management, and acute care. The health center is part of HealthReach Community Health Centers, a system of twelve practices in Central and Western Maine with a 49-year history.
